• IL SB 818 (Sex-ed) Opt-Out Forms. Share forms with friends and neighbors. FYI, NOT all forms are equal. Some are better than others. Be sure to submit the form regardless if the school district has opted out of adopting PA 102-0522 (Sex-ed). A new form must be submitted each year for each child. Mail the form with SIGNATURE REQUIRED or hand submit the form to the school. Ask for the form to be STAMPED as RECEIVED. One copy should go in the student's school file and one should be kept in your personal records.
